- What Examiners See When Operations Fail: The weaknesses that become findings and how to correct them the first time with Jonathan Hughes
Examiners look past the symptom of an operational weakness to its root: management’s failure to identify, measure, monitor, or control the underlying risk. Because findings are written to that root cause, they often read as vague and nonspecific. Management then addresses what it thinks the examiner meant, only to learn it treated the symptom. A good-faith patch is read as a quick fix to a systemic problem, which prolongs remediation and produces fatigue. This session walks operational leaders through that diagnostic so they can interpret what examiners are actually criticizing and correct it the first time.
